This figure of speech is used often. It has to do with reading something. The ‘cover’ is the front and the back of a book. So, this figure of speech means that a person read the entire book. Sometimes, when asked if a person read a particular book, they might answer ‘Yes, but not cover to cover’. (They didn’t read all of it.)
