This figure of speech describes persons who are not practical. Maybe, the persons do not pay attention to things that need their attention. Or, maybe they are ‘day dreaming’ and not concentrating on what is being said to them, or shown to them. You might say to such a person, ‘you really have your head in the clouds.’
